If you have not been in the office for 2 months or more you need a consult with NP which will result in a $45 fee to re-enter the weight loss program.
Once you book if you don't show up to your appointment or cancel within 24hrs of your appointment time. you will be charged a $75.00 cancellation / No Show Fee.
Advanced Lab testing must be completed prior to your appointment. Call our office to schedule or send to your nearest LabCorp.
